REPORT FEES ARE: NO CHARGE FOR FIVE PAGES OR LESS; SIX OR MORE PAGES ARE $0.25 FOR EACH PAGE
Date of Request - The date you are filing the request
Incident Location - The location where the incident took place
Incident Date - The date that the incident took place
Incident Time - The approximate time the incident took place, be sure to indicate AM or PM
Incident Type - Indicate the type of call (i.e. fire, car accident, domestic dispute, etc)
Incident/Case Number – If applicable indicate your Incident or Case Number
Requestor’s Name - The name of the person and/or business requesting the information
Requestor’s Phone - Telephone number where you can be reached for any questions regarding your request
Mailing Address of Requestor, City, State, & Zip Code - Provide the address that should be used to mail your requested material(s)
Information Requested – Place a check beside all that apply, if “Other” be specific as to what information is being requested **Please note that we do not transcribe telephone calls or radio traffic**

There is a $5.00 charge for each audio request, which must be paid by cashier’s check or money order (made payable to York County Treasurer). Payment must be received prior to or upon release of requested audio. There is no charge for the first five pages of any report; reports consisting of 6 or more pages are $0.25 for each page. We will mail all requested material(s) to the requestor’s mailing address.
Please be aware that under South Carolina law, we have 15 business days to respond to your request. This does not mean that we will have the items you requested completed, only that we must acknowledge your request. Although we cannot guarantee a specific date of completion, we will process your request as quickly as possible.
